Title: L'Italia e il federalismo
Abstract: L?Italia e il federalismo (di Amedeo Fossati) - ABSTRACT: The terms of the lively debate on federalism still going on in Italy are presented. Improving efficiency through fiscal federalism is certainly considered, but the dominant theme is perhaps on purely political grounds. Even if the transition process from unitary to federal state has actually started, it is not yet clear what sort of fiscal federalism is being pursued. Co-operative fiscal federalism could be an important vehicle through which northern people?s demand for local autonomy could be made compatible with the less developed economy of southern Italy. To introduce fiscal federalism, however, central government must be willing to reduce its activities. Whilst implementing any reasonable form of fiscal federalism in Italy, the central power would have to considerably change its standpoint and behaviour. In evaluating the achievements towards fiscal federalism up to now, it is possible to perceive an offer of decen-tralisation, which could be an attempt to escape autonomous pressures by building a system of relationships that actually strengthens central predominance. The ideologies of dirigism and centralism appear to be particularly exposed to the danger of using decentralisation as a means of defusing the push for autonomy: this seems to be one of the greatest impediments fiscal federalism is facing in Italy at the moment.

Title: Efficienza tecnica e produzione ospedaliera
Abstract: Efficienza tecnica e produzione ospedaliera (di Daniele Fabbri) - ABSTRACT: The recent reform of hospital financing increased the power of incentives producers respond to. In the meanwhile it made even more necessary to develop tools for productive efficiency analysis in the hospital sector. In this paper we measure technical efficiency of public hospital in Emilia-Romagna, between 1994 and 1995, with DEA (Data Envelopment Analysis). In particular, we investigate how sensitive results are with respect to the definition of output. Moreover we show how to test for significance in efficiency changes, as measured with DEA, by using both parametric and non parametric tests. Results are diffusely discussed and interpreted. The proposed approach, as far as it could lead to useful policy implications, seems definitely relevant.

Title: Organizzazione economica e incentivi in sanit?: il caso dell'attivit? intramoenia
Abstract: Organizzazione economica e incentivi in sanit?: il caso dell?attivit? intramoenia (di Carlo Cambini e Giuseppe Turchetti) - ABSTRACT: The paper addresses the themes of the economic organization and of the role of the incentives of the different actors in the health care sector. In particular, the paper analyzes the peculiar context of the intramoenia regime, according to which health care services are provided within public hospitals by exclusive physicians, but on a market based logic. The paper evidences the limits of the traditional principal-agent approach in addressing the complexity of the intramoeniaregime and suggests the adoption of a more rich and integrated perspective of analysis, able to take into account the articulated contractual relations originating among the different actors.

Title: Capitale sociale e sviluppo economico: un'analisi empirica per le province italiane
Abstract: Capitale sociale e sviluppo economico: un?analisi empirica per le province italiane (di Margherita Scarlato) - ABSTRACT: This work provides some evidence for the Italian regions that social capital, under a broad definition, can have a positive impact on development outcomes. Our empirical analysis includes measures of social capital related to the enforcement of formal norms, the extent of social networks, the provision of local public goods. We show that in the less developed regions of Italy, the so-colled Mezzogiorno, the level of social capital is lower compared to the advanced regions and, at the same time, the productive activities born a smaller share of the cost due to the social capital deficit. This result leads to a ?wrong? incentive towards the underinvestment in social capital and locks the Southern regions in a trap at a low level of both economic development and social capital development. Absent suitable shocks or policy interventions, this out-come is self-reinforcing.
