Two Italian case studies before e during the pandemic Abstract: Purpose. The paper aims to understand if digital healthcare can improve young and chronic patient engagement. The interpretation follows the doctor-patient relationship, investigating whether telemedicine fosters patients? engagement and, if they are young, also of their caregivers. Methodology. The study analyses two Italian digital healthcare cases. A questionnaire and semi-structured interviews are administered to detect information on the new telemedicine service?s satisfaction, utility, and usability. Context. The study shows that e-health can answer the widespread need for well-being without space and time limits, before and during the pandemic, especially in Italy. It allows the humanization of a technology-driven system, which requires strong patient and community engagement to be built simultaneously. Main findings. E-health can be a tool to improve the doctor-patient relationship and create a sense of community among patients. Moreover, patient engagement in a technology-driven healthcare environment is necessary to implement new treatments successfully. Conclusion. We tested new digital healthcare services unique in Italy to understand the point of view of young and chronicle patients and their caregivers to contribute to a bottom-up harmonization process of e-health. This is carried out through institutional measures as well as through spontaneous community behaviour in order to preserve public health and the institutional actions to strengthen the health-care system in dealing with the pandemic. The discussion is framed through the institutional approach. We develop a twostep method. In the first step, regions have been clustered by considering the kind of response to Covid-19, in terms of formal and informal rules. In the second step a random effect panel regression analysis was performed in order to define the effect of the single response variable to Covid-19 on the resilience index over time. Implications for policy makers are then discussed. The first is that coping with a regional emergency requires taking into account regional specificities. Strictly imitative models risk being inconsistent with the characteristics of the area, resulting in outputs that are not homogeneous. The second involves social capital, which is crucial to the application of the norms. In regions with high social capital there is a sense of solidarity that presents itself in informal rules aimed at reinforcing the formal rules. However, there is no sense of conformity for the sake of conformance to the standard. La survey ha indagato le seguenti aree: caratteristiche e parametri dimensionali dell?Azienda; descrizione della governance, malpractice medica/contenzioso; descrizione della strategia economico-finanziaria utilizzata; rilevazione fenomenologica della valutazione del rating professionale aziendale ed interazione malpractice medica-governance clinica. La metodologia di raccolta delle informazioni e dei dati relativi a questi argomenti ha preventivamente tenuto conto della possibile grande eterogeneit? delle diverse articolazioni aziendali nonch? della relativa bassa conoscenza di alcuni termini o fasi operative. Sono presentati poi i dati relativi a 3 temi suddivisi per nord, centro e sud. I principali risultati sono sintetizzati nei seguenti termini: valutazione sinistri, impatto del rischio clinico-sanitario, gestione economica finanziaria, strumenti economico-finanziari e affidabilit? professionale. Assenza di un affidabile metodo di verifica dei risultati ottenuti. In vari contesti ? dichiarata la riduzione della sinistrosit? senza il supporto di evidenze empiriche in quanto questa dinamica pu? essere valutata per un periodo lungo superiore a quello degli incarichi manageriali. Altro aspetto importante sembra emergere dalla non adeguata attenzione su alcuni elementi come le cartelle cliniche e le adeguate expertise professionali nella gestione di processi che hanno un impatto economico rilevante per l?Azienda.
