A further aspect to be considered concerns emphasizing the number of problematic gamblers, a figure that might turn out to be counterproductive (an accurate estimate helps prevention policies) while concealing other correlated issues (economic crisis, loneliness of elderly people, the lack of spaces for socialization), with the risk of transforming important social problems into specific health problems, that is the medicalization of gambling. Here we present the results of six studies conducted in the North of Italy in the period between 2000 and 2015: a social issue emerges, concentrated in particular places (e.g. caf?s, tobacconist?s) where various types of games are available (e.g. slot machines, instant lotteries, scratch cards), and not always related to other types of addiction. Gambling is most widespread among males and increases with age, but factors that seem to affect intensive gambling are social rank (low level of education and non-professional condition), particular lifestyles (being overweight, cigarette smoking, alcohol abuse) and other behavioural disorders (compulsive shopping). It is estimates that there are 1.8 subjects with gambling-related problems who need treatment per one thousand residents, but only one in three turns to the health services, the most serious cases and with other concomitant issues (psychic disorders). This implies the need to promote and make known the services existing on the territory, and the need for a supply of innovative pathways oriented to a variety of targets, together with dedicated prevention initiatives. This paper reviews recent research on adult gambling and problem gambling in Europe (2000-2016). A systematic literature search was carried out using some academic databases, Internet and governmental websites. Following this search and utilizing exclusion criteria, 46 studies on adult gambling prevalence in Europe were identified. These studies demonstrated that there are wide variations in past-year problem gambling rates across different countries in Europe (0.12% to 3.4%). However, it is difficult to directly compare studies due to different methodological procedures, instruments, cut-offs, and time frames. Despite the variability among instruments, some consistent results with regard to demographics were found. Findings highlight the need for continuing monitoring problem gambling prevalence rates in Europe in order to examine the influence of cultural context on gambling patterns, assess the effectiveness of policies on gambling related harms and establish priorities for future research. In a path departing from the economic entity of the phenomenon, concerning both generated income for the State and expenditure by gamblers, through the description of the increase in the offer of gambling opportunities, the paper presents meaningful epidemiological data describing the current features of the phenomenon in Italy and provides the necessary backdrop to interpret it. The results of two studies, European School Survey Project on Alcohol and Other Drugs (ESPAD?Italia), and Italian Population Survey on Alcohol and other Drugs (IPSAD?), conducted by the Institute of Clinical Physiology of the National Research Council, form the basis for an in-depth analysis of the characteristics of gambling both among adolescent students aged 15-19 and in the general population (15-64). The use of specific screening tests allow to provide an insight into the diffusion of problematic forms associated to this activity. The description presented in the present paper comprises a detailed analysis of individual and environmental characteristics associated to gambling behaviours and different gamblers? profiles. Evidences provided can greatly contribute to informing evidence-based prevention policies and interventions, whose effectiveness is being displayed. Indeed, statistical analysis presented in this paper show how the increase in prevention activities implemented in school is associated to a decrease in the share of young at-risk and problem gamblers. In fact, the latter stands as one of the moments of "capture" of the potential gambler, and the context in which he needs to be properly informed, among other things, of the odds of winning and of the risks of addiction. In this respect, an important role was played by the Italian Competition Authority, within the competencies assigned to it by national law on misleading advertising and unfair commercial practices. Several decisions have been adopted by the Authority in this field, highlighting the circumstances where an advertisement may be considered likely to deceive, therefore jeopardizing the economic behavior of the consumers. Panoramica e specificit? della prevenzione strutturale e dei trattamenti residenziali Abstract: In Piedmont region public facilities for addiction started to work with pathological gamblers 15 years ago, while the inclusion of the gambling disorder within NHS in Italy happened only in January 2017. First interventions - in 2003 and 2004 - were outpatient, with activities borrowed from experiences in the clinic of substance use disorders, activated by psychologists, social workers and professional educators without specific fieldwork experience and training. In 2006 in the ASL (local NHS branch) TO3 began the first specific residential treatments in Italy. In 2015, 1569 pathological gamblers were treated by public services for addiction in Piedmont - ten-fold those of ten years before. For structural prevention, Piedmont is the Italian region where there is the highest number of municipalities that adopted provisions to limit gambling supply with slot machines and VLT. The municipalities that implemented such measures are now more than 150, including all the main towns, with a population of 2,5 millions (over 50% of total Piedmont population). We first review the economic paradigms of the Civil Economy and other heterodox approaches, that provide alternative models of human behaviour and anthropology, discarding the classical homo oeconomicus metaphor and its strict hedonism, and proposing concepts such as eudaimonia, relational goods, public prizes and virtues. This work carries out a first comprehensive mapping of this phenomenon in the Marches region, at a very detailed territorial level, investigating the morphology of the infrastructural digital divide and its relations with selected indicators of socio-economic development. A composite picture emerges, that first confirms that the development potential is positively correlated to the digital infrastructure endowment, with the most rural areas being penalized by private telecom operators, not submitted to universal service provisions. However, exceptions and nuanced situations also occur. Taking into account the polycentric and diffused socio-economic model of Marches, the complex morphology of this divide reinforces the seriousness of the regional structural decline.
