Nelle società occidentali gli script sociali definiscono la sessualità maschile come attiva, caratterizzata da un costante ed elevato desiderio sessuale, ed orientata alla prestazione, attribuendo così un ruolo centrale all’erezione. Per effetto di questo orientamento, la sessualità maschile è sempre più oggetto di medicalizzazione, ovvero essa è trattata come patologia nella misura in cui devia da un immaginario stereotipico socialmente condiviso. Nei recenti trial farmacologici l’attenzione è stata quindi incentrata su alcuni aspetti dell’attività sessuale come l’aumento percentuale del numero di eventi definiti "sessualmente soddisfacenti" e questa prospettiva sulla quantità anziché sulla qualità dell’attività sessuale è stata criticata. La difficoltà nel comprendere i vissuti di questi uomini potrebbe derivare da un bias di base, rappresentato dal significato che gli uomini attribuiscono all’espressione "rapporto soddisfacente" con tutta probabilità fortemente influenzato dagli script sessuali dominanti e quindi centrata sugli aspetti quantitativi, piuttosto che emotivi. Tale carenza, potrebbe in una certa misura spiegare il fenomeno di abbandono delle terapie farmacologiche anche quando efficaci. Per questo, la necessità di sviluppare una visione più complessa e ricca della condizione di difficoltà sessuale appare sempre più importante e centrale per la medicina sessuale.;
