The aim of this work is to hypothesize an individual psychotherapy first approach model for spontaneous requests within a systemic point of view. The authors work as teachers of the Milan "Mara Selvini School of Psychotherapy" and collected several supervision questions about individual therapy demand that come from the community care facilities the learners work in. First the real life conditions of the patient must be assessed, then the therapist strain is to catch «who this person is» within his existential framework, trying to follow an individual/relational attachment based way to create a positive therapeutic alliance. Moreover the typical systemic Genogram is affordable, to address the patient’s mind toward a relational thought of his own suffering. Two kind of patients must be expected, relatively to their attachment issue: in the first case the patient shows a fluent and coherent autobiographical competence while the second case shows an impaired verbal attachment memory and drives the therapist to focus on his procedural and implicit "know how" to stay with others.
