The residents of psychiatric structures often raise questions regarding sexuality, creating objective situations of difficulty for the staff who must manage extremely delicate situations from both clinical and ethical points of view. Scientific literature on the subject is scarce. The present contribution represents an attempt to find indications for a method of useful intervention within the organization of a group psychiatric structure. Some critical points are developed using situations taken from clinical practice that seemed paradigmatic and/or descriptive of the seriousness of the situations with which the staff is called upon to cope. In the presentation of clinical cases, some "key points" are highlighted that also call attention to the lived experience of the staff. The sexual questions raised by the patients are often not faced in institutional settings. Excluding the sexuality of the patient from the work of the team means subtracting the instruments necessary for reading one’s own lived experience, one’s own "counter-transference,- with the consequent isolation, solitude and abandonment of the staff member in respect to themes so emotionally involving
