The Author analyses the characteristics of two different types of community psychiatric services - clinical and ethical - that appeared after a period of eclectic practices engendered by the Italian psychiatric reform (1978). Nowadays, psychiatric services have in common professions, functions and structures, but differ in care, team and patient relationships, connections with the community and outcomes. The Author describes theoretically the two different types of services in order to highlight the current mix of practices in community psychiatric services. He also describes practices of inclusion that facilitate the transformation of a predominantly clinical service into a primarily ethical service.
