This paper presents the ongoing methodological debate concerning whose preferences should be taken into account in the health care priority setting based on QALYs. Moreover, by means of an empirical study involving 100 patients, evidence is offered of how the experience of illness may produce contrasting effects on the evaluation of health states, showing that adaptation effects may coexist with effects of opposite sign.
