The Author presents an experience of a home-based psychotherapy carried out with a psychotic patient. Through the narration of the clinical case, he shows the importance of grabbing on a solid internal setting and using one’s own countertransferal feelings as a fundamental compass in a journey fraught with obstacles. The Author highlights three aspects that seems to be specific of the home-based psychotherapy: the impact with the patient’s physical world, the continuous mixing between interior and exterior and the reversal of the figure-ground relationship. Particular attention is given to the excess of reality which impacts the therapist and to the possible interpretative reading modes.
