A diagnostic formulation of a clinical case of a 12-year-old patient is presented which is studied through a continuous symptomatic analysis, which identifies the individual vulnerability, the exposure to early and perpetuated traumatic factors, the occurrence of negative symptoms (social withdrawal, rage explosions, negativism and cognitive deterioration) and the reference idea. This symptomatic pattern is compared with the child’s personal and familiar history and with the attachment figures relationship, considering specifically the features of "protectiveness and control", which are reflected in interpersonal relationships. The separation and the subsequent reunification with the mother and the family acts as a trigger for the development of a related symptomatology associated with disorganization of thought processes. The analysis and the understanding of the symptoms allows the identification of an association between the rigid and persevering thought patterns, the reference idea, the occurrence of negative emotional states, and the development of behaviors constituted by threats and self-hetero-aggressive conduct, including suicidal ones.
