The paper analyses the evolution of community healthcare in EmiliaRomagna in the framework of Ministerial Decree 77/2022. After outlining the main demographic and social trends, it illustrates the regional institutional setup and the introduction of new organizational settings and professional figures, outlining both the continuity with a longstanding tradition of healthsocial integration and the specific features of the EmiliaRomagna model. A section is devoted to local pilot experiences that seek to integrate health services, social services and community resources within a community welfare perspective. The final part analyses the main open issues – teamworking, involvement of general practitioners, limits of proactive care, avoidable hospitalisations, citizens’ disorientation – showing how population ageing makes it urgent to strengthen community welfare and riskstratification tools to achieve a successful transition towards population health management.
