The research focuses on safety and risk management within the piloting service offered by the Pilots Corporation of the Genoa harbour. A case study method has been used, integrating three sources of evidence: technical and procedural documents analysis, interviews based on the Critical Incident Technique, observation. Based on the SHELL model, 53 critical incidents have been analysed and categorised in environment, hardware, software and liveware risks. The results highlight the most relevant risk factors embedded in piloting, as well as the management strategies formally or informally shared within the work system, together with suggested interventions for improvement at immediate and long-term scale.
