Nowadays, most of western countries are facing two main demographic changes: the progressive ageing of the population and the increasing migration flows from non western countries. These two phenomena are impacting differently on population pyramid, increasing the demand for public services and creating potential problems to the sustainability of these services. In this paper we focus the analysis on how these phenomena will impact on the future demand and sustainability of Accident and Emergency Departments (AEDs) in Liguria, providing a forecasting of the total AED demand and expenditure for the next decades (2012-2065). Results show that the demand is estimated to increase impressively over the period considered, especially because of the increase in foreign residents rather than for the progressive ageing of the population. As a consequence policy makers may adopt regional policies specifically addressed to the needs of foreign patients; indeed, foreign patients tend to have more various needs than older patients who generally suffer for chronic conditions and require standard treatments.
